One of the threads, I believe the "What has been found?" thread, has info on Byron Preiss's e-mail, address and phone #. It may be hard to contact him because we have already pestered him very much. You may want to talk to Egbert about that issue because he has the best reason to communicate with Mr. Preiss (Egbert found the Cleveland casque.
However, you can probably ask anyone to snail-mail you a copy of the form. If you do not want to give your name and address on the open board, you can always e-mail someone.

FYI, I am still waiting for B.Preiss to get to the bank and get the jewel.
Since he does not have exact recall of all locations where the treasures are buried, he will most likely need to look in the bank vault to determine if a solution you submit is correct. I believe Wilhouse is waiting to hear from him regarding the Houston treasure.
Certainly, when I do get the jewel, you guys will be the first to know.

Do you have a number to call or an address to mail a solution to? I believe I may have found one of the locations of one of the 10 remaining Casques. I am not however able as of this time to go and retrieve it but I am very confident I have narrowed one of them down to a certain place in the U.S. I too had the book back in 1982 and couldn't as of then figure any of the verses and pictures out... but I think I know which picture and verse go together and where they are pointing to. If there is no mailing address or number... I will wait until I can go to this place and see if it is there. Like you... I would be very happy to have found one after all of these years of frustration and anguish! ;D

If all else fails, you may want to consider trusting someone to check out the location for you. Of course, this is entirely up to you because it does require much trust. By digging through the various threads you could probably figure out who lives in what area of the U.S. and then e-mail that person. Yet, there may be no one near the potential location. Just a thought.

BP has not answered my request for confirmation of a location as of today. I believe that he has not yet gone to the vault to get the answers or the gem.
I have not been waiting around though, and I plan to take some drastic measures to find the Houston cask in the near future. More to come.

So...does this mean that digital reproductions/photocopies of the form will NOT be accepted as valid forms of entry? You MUST have the original form from the book?

From the email I got from Preiss, it doesn't matter if you have digital reproductions or not.
The only thing that will get you a response is a photo of you holding a casque emailed to him.
